What about NoteCase Pro? It can write to a SQL file (ncdb extension) and record sound files. It also handles attachments. Ultra Recall can do even more.

TheBrain could fit your criteria. Yes, it is costly but I think the free version would work for you.
Another choice could be Scrivener. Not what it is designed for, but I think you could make it work...

Perhaps, but it worked for me when I tested it. Sorry you could not test it. No matter because in my opinion there are lots of better programs of this sort.

I understand the confusion. Signing up/in is optional. You can dismiss the dialogue and proceed to use LeaderTask locally. It is free to use in this manner.

If I remember correctly, CintaNotes version 3 should incorporate many additions you are looking for.
