After trying it out a bit, looks positive.
Good editor, fast, stable. Not very automated, so could be used for other than help authoring but may not be as productive for producing help files.

HelpSmith is free today at GAOTD. It seems like a decent 2-pane outliner, capable of generating help files. I'll download it and give a more detailed report later on.

Yes it does seem like a good start, a standard 2-pane outliner with the ability to create HTML-like help files (not real ones). HelpMaker is a similar product but which can generate real help files.

Thanks for the kind words Daly. I think the answer is that it is so easy to combine a simple tree control with MS rich-text control. You then instanly get a 2-pane outliner, like zillion others. Kind ...

It isn't GrandView just yet, but SQLNotes HTML Export has been expanded. Outline and rich text can be combined into a single document with the possibility to collapse both the outline (click on the +)...
