I'm showing my ignorance, but what is a flat-file outliner? Or anyway, how is it different from any other outliner?

flat and attachment have many meanings.
* Rightnote has many attachment types as inline. File export types are many, can be converted to markdown easily via pandoc or import into MD editors. Don't think it has markdown yet.
* QOwnNotes, free, has markdown as native, navigation panel on left shows markdown tags levels as outliner.
In setup, either choose FULL or Full-Vertical before clicking finished to get all the outline panels, properties, tags etc.
Set both interface and editor to dark mode and looks like this:

* But if you are really wanting to get serious, might try Sublime Text (not free). Or better yet either Atom (free) or MS Visual Studio Code (free) then for each:
set up Command Palette (Ctrl+Shift+P) prelims for each
then search in in command palette for keywords to find plugins, i suggest search for:
markdown
attachment
folder (I have a few in sublime/atom/MS VSC, can work as outliner &/or file/folder trees on the left/right panels)
tree
outline
etc
There are tons of plugins for each of the 3 apps. Also tons of nice dark/black themes for each, both for interface and editor styles.
I'd say go with MS VSC, it is getting bigger than the rest with plugins, though you might prefer the old fave, Atom.

Outlinely uses files that are in its own variant of OPML stored in iCloud and supports some markdown formatting (but not all).

Org mode just works with plain text files and easily exports chunks of the outline to markdown (and it's native format is actually better than markdown).

- uses flat file storage (no database)
- native OPML file data format
- save it to your local computer, usb drive, network drive, your own cloud (your data, your file, you decide where you want to save it)
- it supports file attachment as links
- it exports to MD (markdown)

Another option may be https://www.mindforger.com/ though I am not sure whether its native file format is flat. It can export to .md though.

I'm using Joplin more and more as time goes by. It calls itself "Offline First". You don't have to setup cloud in settings, schedule auto sync or press the sync button. Has the other prerequisites stipulated in OP too. Tons of features, including Web Clipper extension for Chrome/Firefox to import web data with formatting & links. The dev and his buds are regularly adding new features. Free, open source, e2e encryption, image display, attachment files links to auto lunch apps and more.

Yep good for quick data sharing, for heavier MD work with multimedia attachments I use Boostnote. Atom & MS VSC have several superb MD preview plugins. dual pane, & cool dark multi-colored themes.
