Hi Cassius,

I'm sorry to hear that you're having problems logging in. The page you're referring to is not to login but rather to change your settings. I'd recommend navigating to :
http://mantis.sqlnotes.net , or
http://mantis.sqlnotes.net/view_all_bug_page.php

A login dialog page will appear where you can enter your account / password. Make sure you check "Save Login" and you'll be automatically logged in.
