A long time ago I trialed an web based app "KustomNote" that was built templated notes (forms). It's been relaunced as "Transpose" -- better name, but has nothing to do with transposition. The site is at

and there are Android and iOS apps. The free account includes 10GB of storage "enough templates to get you started". Paid accounts are $180 USD / year, and provide unlimited templates, storage, and the obligatory collaboration features.
In both accounts, templates can be customized, notes exported (csv and Excel), and forms searched by field. Transpose is a notetaker, sort of, but more like a flatfile database lite.
