Why only U.S.? -- I don't know.
Why they don't tell registered users when an update is available? -- I don't know that either.
NOTE the warning about the Inspiration Libraries folder. (Although I didn't find any change from the 9.03 Libraries folder.)

Strange policy. And, as I gather, you need vs9.1 to be able to exchange outlines with the iPad app.

It's another of my bug-bears with Inspiration.
Not only is the app itself a bit quirky but so is their approach to sales.
They produce two versions of each major release - one for the U.S. market and another so-called 'International edition'.
To make it even tougher for buyers outside the States, they do not sell the IE edition direct. They sell through vendors - at a marked-up price of course.
Weird.

These sites may offer a possible solution; and, as long as you use an intermediary like Paypal for payments, there would be no mismatch between your credit card address and shipping address:
