I know this only too well. I earn my living as freelance writer and translator.
What I do to (try to) avoid this: I format the text in Word or OpenOffice.org Writer to closely match the finished format and give it to my customers. For my normal fee, they usually get 2 revision cycles. In case, they have changes after revision 2, I charge extra.
This works most of the time (sometimes I do 3 revision cycles for a new customer when I want to keep him). But I admit: There have been times that I did more (smaller) revisions - grudgingly.
