The best system I have personally seen is MDE Infohandler’s. It creates a hierarchical list while you are adding keywords and shows you the full keyword “path”; this makes any inconsistencies obvious.

IDEA! uses a category tree; this clearly shows the structure; an information item can belong to any number of categories and it will appear in all relevant “branches”.
