Well, I think that it’s not just a matter of formatting. There should be nothing left to guessing, whether you were in one or the other state. Like, the layout in TreePad or most other outliners don’t look like normal word processing at all. It’s much more dynamic, for use on screen, instead of Word, where the goal is to produce something to print out.

I think that MS should have money enough by now to do some new development of Word, and not reuse the code from 15-20 years ago ;-)
