Granted there are abilities that pertain to one category and not to the other, I’m not sure why you think combining these functions so problematic. It would be problematic if the functions were contraries, but I don’t know of any organizer function that interferes with outlining or vice versa. Of course, ADM 3 is currently trying to satisfy the demands of both, and it contains only about a megabyte of code, less than most straight pims and scarcely more than the minimalist ActionOutline. So despite the number of diverse features required, the sheer volume of code isn’t prohibitive.
What does NoteMap provide that ADM doesn’t because it tries to be a pim as well? OK, here’s one feature that NoteMap has that ADM doesn’t: an extremely rich array of alternative methods to accomplish the same end. How important is that?

I’m sure there are other features that are also necessary, but what I’m trying to say is that I don’t think any piece of software can really do both these jobs perfectly—GrandView came close. Maybe someday, someone will make it possible to perform the functions of NoteMap within an article in a program like Jot or TreePad… seems a lot to ask for, but we can dream!
